Appointments and Check-In Process
To ensure the best care, we work by appointment. We will make every effort to schedule you promptly, either with the first available provider or with the provider of your choice, as appropriate.
Several days before your appointment, you will receive a text and/or email from our check-in platform, Phreesia. Completing your check-in through Phreesia ahead of time will streamline the process and reduce wait times when you arrive at our office.
What to Bring to Your First Appointment:
- A valid photo ID
- Insurance or disability forms (if applicable)
- Your billing insurance information
- Notes or reports from your referring physician
- A list of current medications
- Any X-rays or other imaging studies related to your condition
